Get Your Results

Per Arizona law, you can get your test results directly from us. Please see below for a number of options on obtaining your laboratory test results.

From Sonora Quest Laboratories

  • Our Patient Results Portal can be used to access reports from July 6, 2015 and beyond ONLY. Please see the other options below for reports prior to this date.
  • A User Account is required to access the Patient Results Portal. Create an account
  • Get Your Results
  • You can request your laboratory results from us by completing and submitting an Authorization to Use or Disclose Protected Health Information (PHI)  (also available in Spanish)
  • Beginning July 6, 2015, you can request and pick-up your lab results by visiting our Health Information Management Services (HIMS) office in Tempe. Our HIMS office is located at 1275 W. Washington St., Suite 103-A in Tempe and is open Monday - Friday from 7:30 AM to 4:00 PM. Visitor parking is available in the front of the building. Our HIMS office can be reached at 602.685.5454.
