Medical Plan Choices
You and Laboratory Sciences of Arizona share the cost of any coverage you elect. You pay your
cost for coverage through pre-tax or post-tax payroll deduction. You receive a refund of a portion
of LSA’s cost if you elect no Medical Plan coverage and provide proof of other medical coverage.

Dental Plan Choices
You and Laboratory Sciences of Arizona share the cost of coverage you elect. You pay your cost for
coverage through pre-tax or post-tax payroll deduction.

Legal Plan
You pay the cost of coverage if elected. You pay your cost for coverage through
