We appreciate your cooperation in ensuring everyone's safety when visiting a Sonora Quest Patient Service Center.
- As we are following CDC guidance, please wear a mask or face covering in our Patient Service Centers (bandanna or handmade mask is acceptable).
- If you do not have a mask, talk to the front desk staff and kindly maintain six feet of distance.
- Please avoid bringing minors and guests with you whenever possible. Guests with you that are not minors or needed for support in specimen collection should plan to wait outside of the lobby area in support of safe social distancing.

Sonora Quest is contracted and in-network with the majority of health plans in Arizona. If you have one of the following non-contracted and out-of-network plans, we recommend that you visit the contracted laboratory for services to minimize out-of-pocket costs or view our discounted pay at time of service rates if you wish to pay out-of-pocket.

Important Information Regarding Walmart Drive-Thru Appointments
- An electronically submitted doctor’s order or self-ordered COVID-19 Symptoms/Exposure diagnostic test (ordered through My Lab ReQuest) is required.
- If you have a paper order from your doctor, please click ‘Cancel’ and schedule a Carside appointment.
Important Information Regarding COVID-19 Carside Appointments
- A doctor’s order or self-ordered COVID-19 diagnostic test (click here to order through My Lab ReQuest) is required.
- You will remain in your vehicle and a Sonora Quest team member will come to your vehicle to observe you self-collect a nasal swab sample.
- Additional instructions will be provided in your appointment confirmation email.
Important Information Regarding COVID-19 No Symptom Appointments
- You do NOT have COVID-19 symptoms.
- You have NOT recently been exposed to someone known to have COVID-19.
- A doctor's order or self-ordered COVID-19 No Symptoms diagnostic test (ordered through My Lab ReQuest) is required.
- If you have symptoms or have recently been exposed to COVID-19, please click 'Cancel' and schedule a Carside or Walmart appointment
Important Information Regarding Pediatric Appointments
- Please note that separate appointments are required for each pediatric patient
- If you are scheduling for more than one pediatric patient and are unable to find consecutive appointments, please contact us toll-free at 1.855.367.2778, M-F, 7:30 a.m. – 4:00 p.m. (excluding holidays) to schedule
- Note that you can schedule appointments for you children under your account - you do not need to set up accounts for your children
Sonora Quest Laboratories provides you with the option to schedule an appointment to have a phlebotomist come to your home or place of business for sample collection.
Note that this service is available for blood collection only and is not available for testing requiring other samples such as urine, swabs, etc.
This service is available for a prepayment of $60 and is not reimbursable by insurance. Note that Medicare Advantage members are restricted from this service per Medicare Advantage plan limitations.
I acknowledge that mobile phlebotomy requires a $60 prepayment, is not reimbursable by insurance, and that I am not a Medicare Advantage member.

- Fasting is generally 8-12 hours, however check with your doctor about requirements.
- Glucose Tolerance Tests may take up to 5 hours. Allow adequate time for your visit. For safety reasons, you are not allowed to leave the facility during the course of the Glucose Tolerance Test.
- Semen Analysis: On-site collection is required for Semen Analysis Complete and Semen Analysis Post-Vasectomy with Reflex to Count/Motility testing. Home collections are unacceptable.
- Follow any instructions that your doctor provides you regarding your ordered tests.
- Plan ahead; children must be supervised while at our centers.

- Proceed to the Walmart pharmacy drive-thru and notify the representative that you have an appointment for COVID-19 sample collection.
- Please remain in your vehicle and the Walmart representative will observe a self-collected nasal swab sample collection.

- Please bring your order form (either self-ordered or from your doctor) and your insurance card.
- If you self-ordered a No Symptom COVID-19 test through our My Lab ReQuest process and did not prepay for the test through our website, please note that payment will be due at time of service and you will be required to enter the building to complete the transaction.
- Locate the designated parking space(s) for Carside collections, which will be near the building entrance or, in some cases, in the back of the building.
- Closely follow the instructions on the sign posted in the designated parking space to notify us of your arrival via phone or text using the contact number provided.
- Please remain in your vehicle and do NOT enter the Sonora Quest facility.
- For everyone’s safety, please wear a mask while you wait in your vehicle and until it is time for your sample collection.
- A Sonora Quest team member will come to your vehicle to serve you and will observe a self-collected nasal swab sample collection.

Important Information Regarding Your Mobile Phlebotomy Appointment:
- Our Mobile Diagnostic Services team will contact you by phone at the number you have provided within 1-2 business days to complete the appointment scheduling process and to collect prepayment.
- A Sonora Quest Laboratories phlebotomist will arrive during the window of time you selected for your appointment.
- If there are any unanticipated delays, we will contact you by phone at the number provided.
- If you need to cancel your appointment within the 24-hour period before your scheduled collection, call 602.685.5000 and select option 5.
- If requesting mobile phlebotomy services at your place of employment, please ensure that a private room is available for sample collection prior to your scheduled appointment time.
