Sonora Quest Laboratories will use and disclose your PHI for purposes of billing and payment. For example, we may disclose your PHI to health plans or other payers to determine whether you are enrolled with the payer or eligible for health benefits or to obtain payment for our services. If you are insured under another person’s health insurance policy (for example, parent, spouse, domestic partner or a former spouse), we may also send invoices to the subscriber whose policy covers your health services, as per our Notice of Privacy Practices.

Ages 2-17: Parent or legal guardian must complete the eligibility screener on behalf of the patient and accompany the patient to the appointment. Also note that if you select to access test results via our SonoraQuest.com results portal, you will be required to request access to your child's results under your own account (learn more at SonoraQuest.com/results).

I understand that this test result is required by Arizona Administrative Code (R9-4-302 and 404.H. and R9-6-204) to be reported to the Arizona Department of Health Services (AZDHS) for public health reasons. A local or state public health investigator may contact me for additional information or to ensure proper treatment. If I receive a positive test result, I understand it is my responsibility to consult with my doctor and/or contact my county health department.

If no coverage is available, I understand that I may be charged for the COVID-19 testing service. In-Person in Sonora Quest Patient Service Centers
You have 90 days to cancel your online purchase prior to sample collection at a Sonora Quest Patient Service Center. There is a nonrefundable cancellation fee of $20 which will be deducted from your refund for orders greater than $20. Orders that are $20 or less are not eligible for a refund, although you may still cancel the order, which will also cancel your corresponding appointment.
At-Home Kits
Orders for at-home kits are not eligible for a refund, however, you can still cancel the order. If you choose not to proceed with your at-home testing, we ask that you dispose of your kit(s). We cannot accept returns of unused kits.
Please note that if you have an outstanding balance with us from a separate date of service and your My Lab ReQuest order is eligible for a refund per our refund policy, your refund will be applied toward that balance, with any remaining refund, if available, being processed.
