Patient Ordered Lab Tests

Arizona law allows you to order laboratory testing from a licensed clinical laboratory without your doctor's request. This is referred to as direct access testing and Sonora Quest Laboratories offers a menu of patient ordered lab tests through our My Lab ReQuest™ offering.

There are a number of reasons why you may choose to order your own lab tests. My Lab ReQuest is an affordable and convenient way to order lab tests. No matter the reason, it's nice to know you have the option. The most common reasons include:

  • You want to track lab results for your personal wellness goals

  • You have a chronic condition that requires regular lab tests

  • You want the confidentiality that My Lab ReQuest offers

  • You do not have insurance or you have a high-deductible plan

  • You prefer the convenience and time savings since a doctor's order is not required

Sonora Quest Laboratories believes that it is vitally important that consumers have the most accurate and up-to-date information needed to proactively manage their health and make educated decisions. While we believe the management of personal health is most effective when it involves a partnership with a health care provider, we can help you take a more active role in your health with a menu of select tests that you may request without a doctor's order.

What Tests Can I Order And How Much Do They Cost?

How Do I Order My Own Lab Tests?

You can now order & pay for your tests in 3 easy steps!

You can also order your tests by following the steps below:

  1. Print and complete a laboratory test request form and take it with you to any of our 75 Patient Service Centers located conveniently throughout Arizona. You may also obtain and complete a laboratory test request form at any of our Patient Service Centers*.

  2. While appointments are not required*, you can schedule an appointment on your desktop or mobile device to help save time.

  3. Follow instructions on the laboratory test request form if fasting is required for the testing you are ordering.

  4. Check in with our friendly staff and give them your completed order form. Full payment is due at time of service before sample collection is performed. Note that we accept all major credit cards, checks, and cash.

  5. Once payment is made, a highly-skilled phlebotomist will assist with sample collection and send it to our laboratory for testing.

  6. You should receive your results within a few days, depending upon how you choose to receive them. For your convenience, there are a number of ways that you can get your results directly from us.

*Appointments are required at several of our Patients Service Centers. To check if the location you wish to visit serves patients by appointment only, please visit our Find a Location page. 

Important Notes

  • Only you will receive the test results - Sonora Quest Laboratories will share the test results as required by law and with your physician or other providers only in critical or emergent situations

  • Certain patient test results are required by Arizona Administrative Code (R9-4-302 and 404.H. and R9-6-204) to be reported to the Arizona Department of Health Services for public health reasons. If this occurs a local or state public health investigator may contact you for additional information or to ensure proper treatment.

  • You must be at least 18 years old to request your own laboratory tests or to request laboratory tests on a minor (minor must provide consent on certain tests as required by law).

  • Because a doctor has not ordered these tests, per state law, Sonora Quest Laboratories will not submit to insurance (including Medicare and Medicaid) for payment. Full payment is due at time of service unless you order and prepay online.

  • It is solely your responsibility to promptly discuss all laboratory test results with a physician - neither Sonora Quest Laboratories nor its Medical Director will provide interpretation, counseling, consultation, or care recommendations on the basis of any laboratory results provided to you.
