Sonora Quest Laboratories Now Exclusive with University of Arizona Health Plans and Remains Contracted and In-Network with Cigna Healthcare of Arizona and UnitedHealthcare

Sonora Quest Laboratories is proud to be the exclusive, in-network provider of laboratory services for University of Arizona Health Plans (UAHP).
This exclusive arrangement also includes the following plans:

• Banner University Family Care
• UAHP - Family Care
• UAHP - University Care Advantage
• UAHP - University Healthcare

Cigna Healthcare of Arizona

Sonora Quest Laboratories has proudly served your Cigna Healthcare of Arizona members for nearly two decades as an in-network and exclusive provider of laboratory services.

Sonora Quest Laboratories remains in-network for all Arizona Cigna commercial plan members in 2018 and continues to provide them with high-quality laboratory services.

Sonora Quest Laboratories is out-of-network for Cigna Medicare Advantage Members only. Every effort will be made by our Patient Services staff and Specimen Management Department to appropriately direct any Cigna Medicare Advantage orders to the contracted laboratory.

UnitedHealthcare

Laboratory Sciences of Arizona, LLC has been a local Arizona in-network laboratory service provider for UnitedHealthcare members since January 1, 2007 and continues to be contracted in 2018. Laboratory Sciences of Arizona subcontracts all laboratory services to Sonora Quest Laboratories.

Sonora Quest Laboratories locations can be found on www.myuhc.com under Laboratory Sciences of Arizona.

The only UnitedHealthcare plans that Laboratory Sciences of Arizona/Sonora Quest Laboratories is not contracted with are:

• AARP Medicare Complete (Optum/Lifeprint MA, Phoenix Direct Hospital, and Pinal Networks)
• Community & State Plan/Medicaid (Dual Complete; formerly APIPA)
• Nursing Home Plan (formerly Evercare)
