Sonora Quest Laboratories is the Exclusive Lab for Care1st Members
Effective May 1, 2017, Phoenix Health Plan (PHP) will transition its’ members to Care1st. Although PHP members had the opportunity to select another AHCCCS plan ahead of this transition, the majority will transition to Care1st.
Sonora Quest Laboratories is proud to be the exclusive, in-network provider of laboratory services for Care1st members. Our extensive Patient Service Center network, including nearly 50 locations in Maricopa County alone, is prepared to serve these new Care1st members.
PHP ID Cards
New Care1st members may not receive their Care1st ID cards until mid-May. Beginning May 1, 2017 patients with PHP ID cards should visit Sonora Quest Laboratories for laboratory services.
Healthcare providers: Please note that when ordering laboratory services for these patients, as well as all other Care1st members, please utilize the payor code #4002 - Care1st/PHP Eligibility. PHP member IDs may be used.
Standing Orders
If you have PHP patients transitioning to Care1st with standing orders, please create a new standing order via our Care360® lab orders and results product, your EMR, or paper copies for collection dates beginning May 1, 2017 and instruct these patients to visit Sonora Quest Laboratories for specimen collection.
Patients: Please bring your standing order paperwork with you when visiting any of our Patient Service Centers.
