Attention Cigna HealthSpring members: If you received a bill requesting payment or a letter requesting insurance information and you are currently enrolled with Cigna HealthSpring, please be advised Sonora Quest has identified a claims processing issue with Cigna’s clearinghouse. We are in the process of identifying all impacted accounts and we request that you not submit payment at this time. Once this matter has been fully resolved, a corrected statement will be sent. For online results access, an individual user account and password is required. In using online access for results, you can only access your lab results.
Online results access cannot be used to access lab results for other family members or minor children under the age of eighteen. For lab results for minors under the age of eighteen, please download, complete, and submit the Authorization to Use or Disclose Protected Health Information (PHI) form below to request the test results via email, mail or fax.
Online results access requires a USA-issued social security number and you must be over the age of eighteen. If you do not have a social security number, please call 602-685-5959 Monday-Friday from 8:00am-5:30pm MST for alternative options. If you are a parent or guardian of a minor under the age of eighteen, please see below to fill out and submit a form to request results be emailed, mailed or faxed.

This option should be used to request lab results for minors under the age of eighteen. You can request your laboratory results from us by completing and submitting an Authorization to Use or Disclose Protected Health Information (PHI) (also available in Spanish).You are about to leave the Sonora Quest Laboratories site.
You may be receiving your results before your healthcare provider. This could be because of timing of report delivery or other factors outside of laboratory or provider control. Please allow time for their review.
Your receipt of these results should not be viewed as medical advice and is not meant to replace discussion with your doctor or other healthcare professional. In order to fully understand what your lab results mean, we strongly encourage you to discuss your results with a doctor. Neither Sonora Quest Laboratories nor its Medical Director will provide interpretation, counseling, consultation, or care recommendations on the basis of any laboratory results provided to you.
