Collection Instructions

If your physician has requested that you submit a sample for any of the tests or sample types listed below, we urge you to review the corresponding collection instructions to ensure proper sample collection.

NOTE: Always follow your physician’s collection instructions first - even if they differ from the collection instructions stated by Sonora Quest Laboratories.

Your physician has requested a test or tests that require you to collect all urine for a 24-hour period for the submission to the laboratory for testing. It is important that you follow these instructions carefully to ensure that the test results are as accurate as possible. If you should miss collecting any urine into the container during the 24-hour period, you should discard the entire specimen and start over using a new collection container.

On the beginning of the day of collection, when you rise in the morning, note the time and empty your bladder into the toilet as usual. (Do not save this first urine in the collection container.)
Begin collecting every urine specimen thereafter. It is very important that you keep the container cold, on ice, in the bathroom or in your refrigerator to ensure the best quality results for your test.
It is important to collect ALL urine for the 24-hour period. If you get up during the night, be sure to add the urine to the container. Your physician needs a total volume and all specimens to ensure accurate results.
When you rise in the morning of the second day, collect this specimen into the container as the last specimen of the collection period. Try to arise at approximately the same time as the previous day to reflect collection for a full 24 hours.

EXAMPLE: If you start the collection at 8:00 a.m. on Monday morning, the last collection should be approximately 8:00 a.m. on Tuesday morning.

Label the container with your first and last name, as well as the start and end times and dates for the collection. Keep the container cold and deliver to the laboratory as soon as possible, preferably within 30 minutes. Specimens that are warm will be rejected and you will be required to recollect.

NOTE: Do not begin the collection on a Saturday morning, as there are no Sonora Quest Laboratories facilities open on Sunday to accept the collection.

Please note the following:

Avoid food high in indoles such as avocado, banana, tomato, plum, walnut, pineapple, and eggplant
Avoid tobacco, tea, and coffee for 3 days prior to collection
Avoid foods high in seratonin such as chocolate, avocado, banana, tomato, plum, walnut, pineapple, and eggplant
Avoid medications that include phenothiazines, glyceryl, guaicolote, resorpine, MAO inhibitors, and imipramine. Contact your physician before discontinuing any medications.

Collect the 24 hour urine specimen as follows:

Your physician has requested a test or tests that require you to collect all urine for a 24 hour period for the submission to the laboratory for testing. Special Notes:

Please refrain from eating seafood at least 3 days prior to specimen collection as this will effect testing.
Speak with your physician before testing if you have had a procedure performed in which gadolinium or iodine-containing contrast has been administered.

Collect the 24 hour urine specimen as follows:

On the beginning of the day of collection, when you rise in the morning, note the time and empty your bladder into the toilet as usual. (Do not save this first urine in the collection container.)
Begin collecting every urine specimen thereafter. You may use a commode specimen collection hat to collect the sample. Transfer the specimen from the commode collector into the specimen container immediately and keep the container cold, on ice, in the bathroom or in your refrigerator.
It is important to collect ALL urine for the 24 hour period. If you get up during the night, be sure to add the urine to the container. Your physician needs a total volume and all specimens to ensure accurate results.
When you rise in the morning of the second day, collect this specimen and transfer immediately into the 24 hour container as the last specimen of the collection period. Try to arise at approximately the same time as the previous day to reflect collection for a full 24 hours.

EXAMPLE: If you start the collection at 8:00 a.m. on Monday morning, the last collection should be approximately 8:00 a.m. on Tuesday morning.

It is preferable for the patient to be off medications for three days prior to collection. However, antihypertensives (diuretics, ACE inhibitors, calcium channel blockers, alpha and beta blockers) cause minimal interference. Contact your physician before discontinuing any medications.
Avoid alcohol, coffee, tea, tobacco and strenuous exercise prior to collection.

For Males and Females:

Wash hands thoroughly with soap and water.
Patient should NOT urinate for at least one hour prior to collection of the specimen.
Collect the urine sample in an empty, sterile, plastic, urine collection cup.
Patient must collect at least the FIRST 15 – 20 mL of urine in the cup (about 1/3 of the cup).
Screw the lid on tightly; wipe the container with a paper towel.
Place specimen in the designated area or hand to the office personnel.

Para Hombres y Mujeres:

Standard Urine Specimen Collection

Wash hands with soap.
Open the container WITHOUT TOUCHING THE INSIDE OF THE CUP.
Separate the folds of the urinary opening and use the towelettes to cleanse the area wiping from front to back.
Keep the labia open and begin urinating into the toilet. While continuing to urinate, bring the cup into the stream and continue to urinate until cup is at least ½ full.
Finish voiding into the toilet.
Replace lid on cup. Tighten securely.
Give the specimen to a technician or place in the designated area.

Pacientes Mujeres - Recollection de Orina

Your physician has requested one of the following tests. This is a timed test. You are required to remain at the facility for the duration of the test unless otherwise noted.

One (1) Hour GTT (no fasting specimen required)

You will be required to remain at the laboratory for 1 hour.
You will be given a glucose solution to drink. Finish the drink within 5 minutes and notify the staff members as soon as you have completed the drink. Timing of the test will begin at this time.
Do not eat or drink anything else during this time. A small amount of water is acceptable, but please ask a staff member.
You will be called to the phlebotomy room to have your blood drawn in one hour. Please assist the staff by keeping an eye on time. If the time exceeds one hour and you have not been called, please notify a staff member immediately.
If you feel dizzy, light-headed, or nauseated, notify a staff member immediately. The staff member will assist you by escorting you to a bed to lay down and may place a cold cloth on your head. If you vomit, the test will be discontinued and your physician will be contacted for further instructions.

You will be required to remain at the laboratory for 1 hour.
If directed by your physician, you should fast (nothing to eat or drink except water) for 10 to 12 hours.
Your blood will be drawn for a fasting or first specimen when you arrive at the laboratory.
You will be given a glucose solution to drink. Finish the drink within 5 minutes and notify the staff members as soon as you have completed the drink. Timing of the test will begin at this time.
Do not eat or drink anything else during this time. A small amount of water is acceptable, but please ask a staff member.
You will be called to the phlebotomy room to have your blood drawn in one hour. Please assist the staff by keeping an eye on time. If the time exceeds one hour and you have not been called, please notify a staff member immediately.
If you feel dizzy, light-headed, or nauseated, notify a staff member immediately. The staff member will assist you by escorting you to a bed to lay down and may place a cold cloth on your head. Your blood will be drawn for a fasting or first specimen when you arrive at the laboratory.
You will be given a glucose solution to drink. Finish the drink within 5 minutes and notify the staff members as soon as you have completed the drink. Timing of the test will begin at this time.
Your blood will be collected one time per hour for two or more hours, depending on the testing requested by your physician.
Do not eat or drink anything else during this time. A small amount of water is acceptable, but please ask a staff member.
You will be called to the phlebotomy room to have your blood drawn at one-hour intervals. Please assist the staff by keeping an eye on time. If the time exceeds one hour between draws and you have not been called, please notify a staff member immediately.

Your blood will be drawn for a fasting or first specimen when you arrive at the laboratory.
You will be given a glucose solution to drink. Finish the drink within 5 minutes and notify the staff members as soon as you have completed the drink. Timing of the test will begin at this time.
Your blood will be collected 30 minutes after your finish the glucose solution, then again in 30 minutes after that (1 hour from the time of completing the glucose drink). Then your blood will be collected one time per hour for two or more hours, depending on the testing requested by your physician.
Do not eat or drink anything else during this time. A small amount of water is acceptable, but please ask a staff member.
You will be called to the phlebotomy room to have your blood drawn at one-hour intervals. Please assist the staff by keeping an eye on time. If the time exceeds one hour between draws and you have not been called, please notify a staff member immediately.
If you feel dizzy, light-headed, or nauseated, notify a staff member immediately. The staff member will assist you by escorting you to a bed to lay down and may place a cold cloth on your head. If you vomit, the test will be discontinued and your physician will be contacted for further instructions.

Your physician has requested a special timed test to measure your glucose or blood sugar after eating. Please follow the instructions below carefully to ensure that the test results are accurate.

If directed by your physician, you should fast (nothing to eat or drink, except water) for 12 hours. If this is indicated, your blood will be drawn for a fasting or first specimen when you arrive at this facility.
After drawing the fasting specimen you will be directed to eat a meal, which may include the following foods (amounts listed below) in the sample meal plan. Please note that this is a sample, and you may vary the foods, but be sure to eat a satisfying, varied meal. Your physician may also give you a list of the foods they want you to consume (follow their order, first).
Note the time when you finish eating. Return to this facility no more than 45 minutes after finishing your meal.
Notify the staff when you return to this facility, that you are here to complete a 1 hour Post Prandial / Post Meal Glucose Test. A blood specimen will be drawn at the 1 hour time as you indicate to the staff.
If your physician did not request a fasting specimen, then at _____ o’clock on _____ you should eat a meal following the sample meal plan below, or what is ordered by your physician. Report to this facility no more than 45 minutes following completion of the meal.

Wash hands with soap.
Open the sterile container without touching the inside of the cup or place the commode collection container under the seat lid of the toilet.
Collect sample in sterile cup or commode collection container.
Finish voiding into the toilet, if necessary.
Immediately transfer the contents from the sterile container or commode collection container into the Heavy Metal specimen container.

STANDARD URINE SPECIMEN COLLECTION

Wash hands with soap.
Open the container WITHOUT TOUCHING THE INSIDE OF THE CUP.
Use the towelette to clean the end of the penis. Discard and clean again with the second towellette. If not circumcised, hold foreskin back before cleansing.
Begin urinating into the toilet and while continuing to urinate, bring the cup into the stream and continue to urinate until cup is at least ½ full.
Finish voiding into the toilet.
Replace lid on cup. Tighten securely.
Give the specimen to a technician or place in the designated area.

Oragene DNA Self-collection Kit

Samples are self-collected by the patient:

Before starting, clean mouth by rinsing with water. Wait for at least 1 minute before spitting sample.
Spit saliva into container to “FILL TO” line.
Hold container upright. Close funnel with big white cap. When closed, liquid in the big white cap will be released to mix with saliva.
Hold container upright. Unscrew funnel from tube (by twisting counter-clockwise).
Close tube with small orange cap.
Mix 5 times.
Throw out funnel and big white cap.

Oragene kits are available at the physician’s office. Patients can collect saliva in the physician’s office, at the Patient Service Center or at home.

Specimen minimum: 1 ml of saliva (reach the black line in the collection vial). 1 ml of Oragene added when cap is locked. Minimum total volume 2 ml.

Transport: Whole Blood, Saliva, and Cultured Cells are shipped ambient (18’-26’C). Do not refrigerate or freeze Cultured Cells. Do not freeze Whole Blood or Saliva.

Instructions on application of Pediatric Sterile Urine Collector:

Open package and set aside.
Separate infant’s legs. Make sure pubic area is clean and dry with no lotions to allow adhesive to stick properly.
Peel off backing paper exposing adhesive.
This is a sterile bag. Do not place fingers inside the opening of sterile bag.
Press adhesive firmly without puckering to the skin surrounding the genital area with the excess of bag toward infant’s feet.
Reapply diaper if necessary over bag.
When removing the bag, take care to avoid spilling and contamination.
Holding bag upright cut off the adhesive portion and empty bag contents into a sterile cup provided.
Label specimen with the patient’s last/first name, date of birth, date of collection and time.
Return the sterile cup back to facility for testing.

Specimens should be collected early in the morning, prior to emptying the bowels. Avoid the use of creams or powders prior to collection of the sample.

For Pinworm Paddle:

Remove the cap on the collection container. In the cap you will find an optically clear paddle with one side coated with a non-toxic, mildly adhesive material. This is marked “sticky side”. DO NOT touch this surface with your fingers.
Carefully print the patient’s name with pencil on the end of the paddle.
Press the sticky surface against the anal skin with moderate pressure and remove.
Replace the paddle in the collection container.
Print the patient’s name on the collection container, as well as the time and date of collection.
Return the sample to the laboratory as soon as possible. Please keep the sample at room temperature, avoid extreme heat and cold.

Alternate Collection – Glass Slide:

As an alternate to the Pinworm Paddle, a glass slide with a frosted end and scotch tape may be used for collection.
Write the patient’s name with pencil on the frosted end of the glass slide.
Using a small piece of clear scotch tape, press the sticky side against the anal skin with moderate pressure and remove.
Carefully place the tape, sticky side down on the glass slide and press lightly to ensure that the tape is adhered to the slide.
Place the labeled slide in a transport container and label the container with patient name, date, and time of collection.
Return the sample to the laboratory as soon as possible. Please keep the sample at room temperature, avoid extreme heat and cold.

Your doctor has ordered a test that requires the collection of saliva. These instructions will help ensure we get the best specimen possible for accurate testing. This specimen may be collected at home at the time(s) specified by your physician. Use a separate kit for each collection if there is more than one time specified. Contact your physician if you have questions about the test.

Instructions for Collection:

Do not have anything in your mouth for at least 60 minutes before starting the collection. Do not brush or floss prior to collection. Do not collect a sample if your mouth is bleeding. If desired, you may rinse your mouth with water 10 minutes before the collection. (Figure 1)
Wash and dry hands. (Figure 2)
Allow saliva to start to collect in the mouth.
Open the cap of the Salivette® tube and place to the side. (Figure 3)
Remove the roll swab from inside the Salivette® tube. Do not remove the smaller plastic insert inside the tube. (Figure 4)
Carefully place the roll swab inside your mouth and begin to roll it around. You may tip the tube up to drop the roll swab into your mouth on top of or beneath the tongue. Do not place the roll swab between the cheek and gum. Do not chew on the roll swab. Be careful not to choke on or swallow the roll swab. (Figure 5)
Continue to move the swab around in your mouth for at least 2 minutes or until you can no longer produce additional saliva without swallowing. The roll swab should be completely wet before proceeding to the next step. (Figure 6)
Remove the wet roll swab from your mouth and immediately place it into the smaller plastic insert inside the plastic tube. (Figure 7 and 8)
Firmly place the cap back on the plastic tube. The tube should look like it did before you opened the cap. (Figure 9)
Label the tube with your full name, date of birth and the date and time of collection.
If your physician ordered more than one collection at different times, follow the same procedure for each collection. Be sure to label each tube with the time it was collected.
Return the labeled specimen(s) to the laboratory as soon as possible on the following day.

Important Notes:

Do not use this product on children under the age of 3 or patients with increased risk of swallowing or choking.
The cotton swab is a natural product. Discoloration and dark inclusions or spot(s) are of natural origin and cannot be avoided.

Continúe moviendo la torunda por la boca durante al menos 2 minutos o hasta que ya no pueda producir más saliva sin tragar. La torunda debe estar completamente mojada antes de pasar al siguiente paso. (Figura 6)
Retire la torunda húmeda de la boca y colóquela inmediatamente en la pieza pequeña de plástico que se encuentra dentro del tubo. (Figuras 7 y 8)
Vuelva a colocar firmemente la tapa en el tubo de plástico. El tubo debe tener el mismo aspecto que antes de abrir la tapa. (Figura 9)

For successful diagnosis of infection through testing of sputum samples, it is important that the patient understand the importance of collecting an adequate sample. Samples must be produced from a deep cough. If the sample is contaminated with cells from the mouth or saliva, it will be rejected and the patient will be required to recollect the sputum specimen.

Please follow these instructions to help insure that an adequate specimen is collected:

Brush teeth, gargle, and rinse mouth with water. DO NOT use mouthwash.
Collect the first morning specimen if possible, since it represents a pool of material that would have collected overnight.
Cough deeply, or expectorate directly into a sterile screw-top container. Be careful not to touch the inside of the container or the inside of the lid.
The volume of the specimen need not be large, but must be mucous-like material from a deep cough. The best specimen should be about 2 to 3 mLs or approximately 1/2 teaspoon.
Saliva is unacceptable and will be rejected by the laboratory. The patient will be required to recollect the specimen if saliva is present.
Keep the specimen refrigerated and transport to the laboratory as soon as possible, preferably within 30 minutes of collection. Avoid exposure to extreme temperatures.

NOTE: For Cystic Fibrosis specimens, a swab may be used to collect the specimen. This should be determined by the provider and would be collected by the physician office.

NOTE: For certain tests, the physician may require collection of more than one specimen, collected on different days. If this is required, each specimen should be delivered to the lab on the day it is collected.

Follow physician instructions for collection frequency, usually one or two specimens are collected.

You should be given a clean, screw top container for the collection of each specimen requested.
For each collection, pass a small amount of fresh stool into a clean, dry container. Take care not to contaminate the stool with urine or toilet water. If collected in a diaper, line the diaper with plastic prior to avoid absorption.
Open the container and using a wooden stick, place small scoops of stool into the container. Select areas that appear bloody, slimy or watery.
Check all containers to be certain they are tightly closed.
Label each container with your first and last name and the date and time collected. Place the containers into the specimen bag and refrigerate or place on ice until transported to the laboratory.
Thoroughly wash your hands and discard all supplies in the trash. Excess specimen should be discarded in the toilet.
Return the containers to the laboratory with any accompanying paperwork as soon as possible.

NOTE: You may also receive additional containers that contain liquid preservative. Collect these specimens following the instructions provided with them. If you are collecting both types of specimens, collect the specimen in the empty cup on the last day and deliver to the laboratory as soon as possible.

There are no specific dietary or medicinal restrictions prior to collection of this test unless normally designated by your physician.
If you have active bleeding due to hemorrhoids, fissures, or other conditions, do not use the Insure® test. Contact your physician for additional instructions.
If you are menstruating, you should not perform the test during, or 3 days before or after, your menstrual period. If someone else in the household is menstruating, be sure to flush the toilet prior to beginning specimen collection.
Remove any deodorizers/cleaners/fresheners from the toilet and flush twice prior to collection as these can decrease the sensitivity of the test. If a rust ring is present in the toilet and cannot be removed, collect the specimen in a different toilet.
Follow physician instructions for collection frequency. If none are provided, the laboratory recommends two samples collected from the same bowel movement.
Please be aware that the specimen must be received at the laboratory as soon as possible, but no later than 14 days after the collection date and before any expiration date listed on the test card itself.
Your collection kit contains instructions for use, Test Card, Brush Kit (consisting of two brushes) and two waste bags, postage paid return envelope and a Reply Form with label stapled to a Sonora Quest Laboratories requisition form and/or a copy of your physician’s order and other attachments as required for testing and billing purposes.
Begin by flushing the toilet. After the bowel movement, DO NOT place toilet paper in the toilet bowl. Instead, use one of the blue waste bags provided. DO NOT FLUSH the toilet after your bowel movement. The test is designed to be collected using stool specimen in water.
Lift the flap marked “LIFT HERE FOR SAMPLE” on the test card to uncover the small white squares marked “1” and “2”.
Using one of the blue brushes, gently brush the surface of the stool in the water for about 5 seconds. Transfer a sample of the “WATER ONLY” by gently dabbing the bristles of the brush onto the small white square labeled “1” on the test card for about 5 seconds.
Using the second blue brush, repeat step 4 and transfer a second “WATER” sample to the test card by gently dabbing the bristles of the brush onto the small white square labeled “2” for about 5 seconds. Discard the used brushes in the other blue waste bag and throw in your trash.
Print your name, date of birth and the date sample was collected on the removable label. Ensure the label is affixed to the front of the Test Card (inside the white dotted lines) and your name, date of birth and date of collection are written on the label.
Complete the Reply Form and place the Test Card/Reply Form/Pre-Printed Test Requisition or Physician Order in the postage paid mailing envelope provided.
Return via US mail immediately using the provided envelope or take to any Sonora Quest Laboratories facility, but ensure delivery within 14 days of the specimen collection date.

Follow physician instructions for time period of collection – 24, 48, or 72 hours. You should be on a diet including 100 grams of fat per day for three days prior to collection and during collection.
You will be provided with a one-gallon container or plastic jar with a tight lid. Specimens collected in a metal paint can will be rejected.
Pass stool into a separate collection container and transfer or collect directly into the container provided. Take care not to contaminate the stool with urine or toilet water. If collected in a diaper, line the diaper with plastic prior to avoid absorption.
Before submission, check the container to make sure it is tightly closed and label with your first and last name, the date and time collected, and the total collection time – 24, 48, or 72 hours. Place the containers into the specimen bag and refrigerate or place on ice until transported to the laboratory.
Thoroughly wash your hands and discard all supplies in the trash.
Return the container to the laboratory with any accompanying paperwork as soon as possible after completion of collection.

Fecal Fat, Qualitative

You should be given a clean, screw top container for collection. Pass a small amount of fresh stool into a clean, dry container. Take care not to contaminate the stool with urine or toilet water. If collected in a diaper, line the diaper with plastic prior to avoid absorption. Do not ingest castor oil or mineral oil prior to collection.

Before submission, check the container to make sure it is tightly closed and label with your first and last name and the date and time collected. Place the containers into the specimen bag and refrigerate or place on ice until transported to the laboratory.
Thoroughly wash your hands and discard all supplies in the trash. Excess specimen should be discarded in the toilet.
Return the containers to the laboratory with any accompanying paperwork as soon as possible.

Patients using antacids, anti-diarrheal medication, or laxatives should discontinue use seven (7) days before collecting specimens.
Your physician should provide basic instructions for collection. If none are provided, the laboratory recommends one specimen collection but your physician may order 2 specimens. If so, these must be collected on separate days. Allow 2 days in between collections. A separate kit must be used for each collection and collections should never be combined. If one collection does not fill all containers provided, a separate collection that day may be used to fill the remaining containers. Be sure to note the collection date and time on each container.
Your kit consists of several containers, all or some contain liquid. You may have more than one kit, one for each collection day. Do not drink the liquid and keep out of reach of small children.
For each collection, pass a small amount of fresh stool into a separate clean, dry container. Take care not to contaminate the stool with urine or toilet water. Do not pass the specimen directly into the tubes. If collected in a diaper, line the diaper with plastic prior to avoid absorption.

Stool must be transferred to containers immediately after collection, following these instructions (the laboratory cannot accept specimens that have not be transferred immediately as the test will not be valid):

Open each of the tubes containing the liquid. Using the collection spoon built into the lid, or a wooden stick, place small scoops of stool into the container. Select areas that appear bloody, slimy or watery. Add stool to each container in the kit until the volume reaches the red line marked on each container. Please note: The pink and gray containers must both be filled from the same stool collection as they make one sample for one test.
Carefully mix the contents with the spoon and tightly twist the cap back on the container. Shake the tube vigorously until contents are well mixed.
Fill each tube in the same way being sure to add stool to all tubes in the kit. Check all tubes to be certain they are tightly closed.
Label each container with your first and last name and the date and time collected. Check the box on the label, which looks most like the specimen collected. Place the containers into the specimen bag and keep at room temperature.
If your kit includes containers that do not contain liquid, add stool to these containers with a wooden stick and keep refrigerated or on ice. Kits with these containers should be returned to the lab within 30 minutes of collection.
If you have only kits containing liquid, these should be returned within 24 hours of collection. If you do collections on multiple days, be sure that each kit is clearly marked with the date and time of collection. Do not collect more than one kit in a 24 hour period. Use a separate kit each day and never combine specimens.
Be sure to wash your hands after handling the specimens and containers. Discard all supplies in the trash. Discard any excess specimen should be discarded in the toilet. if you accidentally spill any of the liquid, clean it up with a paper towel and discard in the trash. Contact the laboratory to pick up a new collection kit.
