Insurance

Your insurance policy is a contract between you and your insurance company. It is your responsibility to understand your plan’s specific rules regarding covered services, approved providers, referrals, authorizations and out-of-pocket payments. Please call your health plan with any questions regarding your plan or benefits.

Proof of Insurance

  • It is your responsibility to make sure your health insurance is active before scheduling laboratory testing and to inform Sonora Quest Laboratories staff of any changes to your health insurance.

  • It is important to have your correct insurance information available to help us correctly bill your insurance. You will be responsible for any charges incurred if the information provided by you or your ordering provider is not correct.

  • Please bring your insurance card(s) with you to each visit, and ensure that your ordering provider has your most recent insurance information on file.

Medicare Patients

  • Medicare may not cover all of the laboratory testing that your provider recommends.

  • Medicare requires that we notify you in advance when we have reason to believe that certain tests may not be covered, based on the diagnosis information provided by your provider. In those specific instances, you will be notified prior to receiving laboratory testing and will also be given an Advanced Beneficiary Notice (ABN) explaining which laboratory testing may not be covered. You will have an opportunity to decline testing if you do not agree to accept financial responsibility for the non-covered tests. 

  • For testing that does not require Sonora Quest Laboratories to provide you with an Advanced Beneficiary Notice (ABN), you will be financially responsible for any tests not covered by Medicare. Please call Medicare in advance with any questions regarding your plan or benefits.

  • If you have a secondary insurance to cover these costs, please let us know at the time of service.

Accepted Insurance Plans

  • We have contracts with many insurance companies. View a full list of our contracted health plans.

  • If you are enrolled in a health insurance plan that does not contract with Sonora Quest Laboratories, you can still receive laboratory testing from Sonora Quest Laboratories with an approved prior authorization or by prepaying for your laboratory testing.

Prior Authorization

  • It is your responsibility to know if your insurance company requires you to have a prior authorization to obtain laboratory testing. If you are not sure, please contact your insurance company to review the details of your plan.

  • Please make sure you have the required prior authorization documentation prior to obtaining laboratory testing.

No Insurance / Self-Pay

Patients with no insurance (self-pay patients) are required to pay for their laboratory testing in full prior to services being rendered. View our discounted pay at time of service pricing

We describe Self-Pay as:

  • A patient who cannot provide proof of valid health insurance and there is none on file

  • A patient who inadvertently provides inactive health insurance at the time of service will be sent a bill after the completion of their laboratory testing

  • A patient who requests laboratory testing that is not covered by his or her insurer

  • A patient who is enrolled in a health plan that is not contracted with Sonora Quest Laboratories and does not have a required prior authorization.

Payments Due at Appointment Check-In

  • Co-payments will be collected at the time you check-in for your appointment.

  • Insurance deductibles, co-insurance, co-payments and other insurance fees that are not collected at the time of service will be billed to you.

  • Self-Pay patients, including patients ordering their own testing from our My Lab ReQuest menu of laboratory tests are required to pay for their laboratory testing in advance. Full payment of estimated charges is due before laboratory testing.

  • If a previous balance exists, payment will be collected before any new laboratory testing are performed

  • We accept cash, checks, money orders, and major credit cards including VISA, MasterCard and American Express.

Billing

  • If you owe additional money after your visit, you will receive a billing statement.

  • All balances are due in full within 20 days of the date on your billing statement.

  • If you cannot pay the balance in full within 20 days, please contact our Customer Service Department at 1.800.853.4288 between the hours of 7 a.m. - 12 p.m. or 1 p.m. – 5 p.m., Monday through Friday to see if you qualify for other payment plans or options.

Non-Payment

  • Any unpaid balance must be paid by the due date on your billing statement or your bill will be turned over to a debt collection agency.

  • Non-payment may affect your ability to receive new laboratory testing.

Refunds

  • If you make an overpayment on your account, the overpayment will be applied to your outstanding balance.

  • If there is no outstanding balance, the over-payment will be refunded to you by mail or if paid via credit card, to the original form of payment.

Payment Plans

Sonora Quest Laboratories offers in-house payment plans which grant patients up to five monthly installments to pay off the balance owed. We've also partnered with CarePayment to offer financing options to patients who would like to consolidate multiple personal and family accounts into one affordable monthly installment over an extended period of time.

Patient Financial Assistance Programs

Does Sonora Quest Laboratories offer any programs for patients who need alternative options for paying their bill or may be experiencing financial hardship?

For patients who qualify based on the Federal income poverty guidelines, Sonora Quest Laboratories offers a sliding scale Patient Financial Assistance Program. For additional information please call our customer service department at (800) 853-4288 between the hours of 7 a.m. – 12 p.m., and 1 p.m. - 5 p.m., Monday through Friday.

Does Sonora Quest Laboratories offer a payment plan if I have multiple accounts, or if I am unable to pay the full amount I owe all at once?

Yes. At Sonora Quest Laboratories we understand the importance of making health care services affordable to our patients. We offer patients an option to pay for their medical expenses interest-free over an extended period. Patients can start a payment plan in a few easy clicks at pay.sonoraquest.com or by reaching out to our customer service team at (800) 853-4288 between the hours of 7 a.m. – 12 p.m., and 1 p.m. - 5 p.m., Monday through Friday.

Payment plans for Sonora Quest Laboratories are interest-free, have no impact on your credit, and are available without an application regardless of insurance coverage, credit history, or employment.

How can I apply for the Sonora Quest Laboratories Patient Financial Assistance Program?

The Sonora Quest Laboratories financial assistance program is available to patients with or without insurance.

As a reminder, the patient financial assistance program qualifications are based on total household income and the number of persons in the household.
