I agree to provide accurate and complete information during the registration/identification verification process. I understand any information I provide will only be used to authenticate my identity for the purposes of determining which laboratory results will be available to me in the Sonora Quest Laboratories Patient Results Portal. I understand and agree that I will only have access to my laboratory results. I understand and agree that, in some cases, registration/identification verification may not be authenticated, and I may be instructed to contact the Sonora Quest Laboratories Health Information Management Services department for assistance. Furthermore, I understand that the Sonora Quest Laboratories Patient Results Portal can only be used to access laboratory results from dates of service August 1, 2015 and beyond.
I understand that any laboratory results provided to me through the Sonora Quest Laboratories Patient Results Portal are for informational purposes only. I understand that it is solely my responsibility to promptly discuss all laboratory results with a physician and that neither Sonora Quest Laboratories nor its Medical Director will provide interpretation, counseling, consultation, or care recommendation on the basis of any laboratory results provided to me.
Important Note: Sonora Quest Laboratories utilizes an industry-leading, third-party service for the secure validation of your identity. Information provided during the identification validation process is not available to or retained by Sonora Quest Laboratories.

We post all results as soon as all of the testing has been completed. Most tests are complete within 1 to 3 days, but some tests are more complex and can take longer. If it has been more than 14 days since you had your lab tests submitted please contact us at PatientSolutionsTeam@SonoraQuest.com or by phone at 602.685.5959.
- First Time Users: It may take up to 60 minutes for your results to display.
- Parents/Guardians Requesting Access to Minor Results: It may take up to 60 minutes until you are able to request access to minor results. Please wait 60 minutes and then visit https://test.sonoraquest.com/patient/request-linked-minor/ to submit your request.

Lab test results for minors under the age of eighteen are not available online. To request lab test results for minors under 18, please download, complete, and submit the Authorization to Use or Disclose Protected Health Information (PHI) form available on our My Results page.
